A document viewer and editor for the Claude Code TUI: /doc opens a Markdown file as a navigable document in a pane.

A Claude Code plugin whose mod opens a Markdown file as a navigable document in a pane beside the transcript: an outline, folding, find, links followed in place, and edits that go through Claude and show what changed.
The reader. /doc <path> opens a Markdown file in a pane; path:N and path#heading open it at that line or heading, and the path after /doc completes from the session's directory. Claude can open a file with the plugin's show tool, and every Read, Write or Edit row that names a Markdown file gets an open link under it. A file that is missing, over 4 MiB or not UTF-8 text is refused with the reason in the plugin's status row.
When the session already has a /doc, such as your own doc skill, the engine refuses the mod's, and the mod registers /doc-editor instead, with the same arguments, completion and behaviour. The status row says so: /doc is taken by another command (<reason>); use /doc-editor. Your own /doc keeps running as before. If /doc-editor is refused too, the status row says that, and Claude can still open a file with the show tool. Each registration at session start stands alone, so a refused command never stops the show tool, the watcher or the pane.
The pane draws only the blocks in view, so a document of any size up to 4 MiB stays inside the engine's draw limits. Fences draw as highlighted code, callouts as bordered boxes with their label, and a table too wide for the pane as one card per row. Images draw as their alt text and path. The header shows the path, the heading path, the mode, the read time and the find count.
| Key | Action |
|---|---|
| Up, Down | Previous or next block |
| PageUp, PageDown, wheel | Scroll |
o | Outline; Up and Down (or Tab and Shift-Tab) move between entries, Enter jumps to one |
z, a | Fold the current heading; fold or unfold every heading |
d, u | Show only the current section; back up one level |
f | Find; Enter goes to the next match, b to the previous |
| Tab, then Enter on a link | Open a file or heading in the repository in place; b goes back |
g | Go to a path, :line or #heading |
t, q | Top; close |
j, h | Next or previous heading |
c | Ask Claude to change the focused block |
e | Edit the focused heading, list item or table row on one line; Enter saves |
| Tab, then Enter on a checkbox | Tick or untick that task and save |
n, p | Next or previous block Claude changed |
v | The turn's diff; n and p step turns, v or q go back |
k, x, r | Keep, discard or refine Claude's change to the focused block |
| ctrl+x tab | Give the keyboard back to the pane after Escape handed it to the prompt |
The pane's keys work while the pane holds the keyboard. Escape hands it back to the prompt, so the next letter is typed there; ctrl+x tab (the engine's pane focus chord) or a click on the pane gives it back, and so does running /doc with no path, which shows the open document again with the keyboard on it.
When the open file changes on disk, or after Claude's Write or Edit, the pane re-reads it and keeps the focused block, the folds and the open section. The design is in docs/designs/doc-editor-design.md.
c asks Claude to change the focused block. The pane sends Edit lines <start>-<end> of <path>: <instruction> as your own prompt. The section heading and the block's first words follow on their own lines, each quoted as text from the file: on one line, at most 120 characters, inside a code span nothing in it can close, and marked as not an instruction, so a document cannot write words that read as yours. A status line under the block says Claude is editing it, or that the request is queued while a turn runs. When the file changes during that turn, every block it changed gets a marker in the margin, and the focused one shows keep, discard, refine and view-diff actions. v shows the turn's unified diff, one turn at a time. x writes the block's text from before the turn back as a new edit, and refuses when the block changed again since. A change is found again after a reload by its lines and their neighbouring lines, never by equal text alone, and when a copy elsewhere matches as well x refuses and says so. k keeps the change and clears its marker. r opens the request again with its instruction filled in. The markers last for the session, and a reload of the mod keeps them: the session state holds, per file, the changes whose lines still read as their turn left them, at most 50, the newest kept. A kept, discarded or since-edited change leaves it.
e opens a one-line field on a heading, a list item or a table row; Enter saves it. A text with a line break is refused, since the field edits one line. A list's task checkboxes are buttons under the focused block, beside its link buttons: Enter on a checkbox ticks or unticks it and saves, and Enter on a link follows it. Tab moves the engine's focus ring onto those buttons, and while the focused block has any the hint row says "Tab: next button · Enter: open or tick it". Every save goes through the safe-write path below.

The full install note, with requirements and the test gate, is docs/procedures/mod-install.md.
The plugin's one hooks module is hooks/register.ts, and every call to the mods API sits there. The block below is the complete list of hooks it registers and calls it makes, in the form claude plugin validate --json plugins/doc-editor reports them. The repository's gate fails when the two differ in either direction, so a change that adds or drops a hook or a call updates this list in the same commit.

The plugin registers no tool.check hook, calls no model and makes no network request. It submits a prompt only when you press c or r and send an instruction, as your own words, and it observes the main loop's turn.start and turn.complete to learn which turn ran that prompt. It answers one tool.call, for its own registered show tool. Its other tool.call hook observes Write and Edit: it passes each call on and returns the tool's own result, then re-reads the open file if it changed. Its ui.render hooks on ToolUse and ToolGroup draw the row the chain beneath returns and add one line under it.
The same gate enforces this safety floor whatever the list says, over both the validate report and the sources. It fails, naming the hook, call or line, on any of these:
tool.check hook;tool.call hook other than the observed one, unless it answers a tool the plugin registers with $.tool.register under the mcp__doc-editor__ prefix;tool.call handler, or its .catch handler, that returns anything but the result of next(...), or that does anything with that result but return it;$.model.*, $.agent.spawn) or a network call ($.http.*, $.mcp.*);fetch, WebSocket, XMLHttpRequest or EventSource, free or read through globalThis, self or window or a name bound to one of them (const g = globalThis; g.fetch), resolved by scope so a local function named fetch passes; a use of a global object that loses track of what it reaches (destructuring it, passing it on, a computed key that is no string constant); or an import of a module that reaches the network or starts processes, such as node:http, node:net or node:child_process, in any source the plugin ships, bin/ included;$.process.run other than node --version or node on a ${$.plugin.root}/bin/<name>.mjs script the plugin ships, so no node flag such as -e or --import, and no argv that is not a literal array;$.process.spawn.Every save goes through bin/save.mjs, a dependency-free helper the mod runs with node. It writes a temporary file beside the target and renames it over the target only if the target's mtimeMs and size still match the ones the mod read. When the file changed since it was read, the mod first asks whether to reload, overwrite or cancel. Without node on PATH the mod writes the file directly with $.fs.write, which is not atomic, and says so once per session in its status line.
hooks/register.ts: the hooks module, registered through hooks/hooks.json.hooks/pane.ts: the reader pane, with hooks/header.ts (the header row), hooks/hints.ts (the key bindings and hint row) and hooks/tables.ts (tables fitted to the pane or as cards).hooks/commands.ts: /doc and its path completion; hooks/tools.ts: the show tool; hooks/rows.ts: the open link under tool rows. Each takes the pane or plain closures, never $.hooks/review.ts: the requests sent to Claude, matched to the turns that ran them, and the changes each turn made, with their markers.types/doc-editor.d.ts: the plugin's contract, the session state it keeps (changes, per file).hooks/save.ts: the safe-write path, and hooks/watch.ts: the reload watcher. Both take plain closures from register.ts, never $.bin/save.mjs: the write-then-rename save helper.core/: the shared core, plain TypeScript with no DOM, no Node, no timers and no $. It imports only relative paths. core/types.ts holds the document model.hooks/*.test.ts: tests that claude plugin test runs in the mod environment.hooks/register.ts 197 lines1import type { On } from "claude-code";

3//
4// Block ids follow the heading tree: a heading is `h` plus its position among its siblings at each
5// level (`h2.1` is the first subheading of the second top-level heading), and any other block is
6// its section's id plus its ordinal in that section (`h2.1:3`, or `h:1` before the first heading).
7// An id therefore stays the same while the block's place in the heading tree is unchanged, however
8// its text or the lines above it change. List items and quote content are `children`, with the
9// parent's id and their ordinal (`h2.1:3.2`).
10//
11// The drawn text differs from the source where the Phase 0 spike measured that the engine's
12// `Markdown` element would show syntax as text: task markers become checkbox glyphs, HTML comments
13// are dropped and inline HTML is converted, images become their alt text and path (D-14), a link
14// whose href may not be drawn as a hyperlink becomes its label and its target as code, and a
15// reference link is resolved, since each block is drawn in its own element.
